What is Community Health Integration (CHI)?

Community Health Integration (CHI) is a transformative program designed to enhance patient care by addressing the critical link between social determinants of health (SDOH) and medical outcomes. Through CHI, we empower healthcare practitioners to integrate vital community-based support directly into a patient's treatment plan, fostering a more holistic and effective approach to well-being.

CHI services focus on identifying and addressing unmet social needs that significantly impact a patient’s ability to manage his/her health conditions. These services are provided by trained auxiliary personnel, such as Community Health Workers (CHWs), under the general supervision of a physician or other billing practitioner. By connecting patients with essential community resources and offering personalized support, CHI helps overcome barriers to effective diagnosis and treatment.
